The 2023 - 2024 sports season is approaching. All parents and students are required to update their Final Forms online yearly. Please login to your Final Forms account. Once you are logged in, you will see your students. Please click on the large blue box that says "click here to switch to the 2023- 2024 school year.”  Once you get started you will verify all the information you inputted last school year.  Please make any changes necessary. Once complete, Final Forms will send an email to your student(s) to sign into their final forms account. The student(s) will need to review information and sign each page.
